Client Information and Guidelines at Humantality Counseling

Patient's Rights:

Your engagement in our therapeutic relationship is entirely voluntary, and you retain the autonomy to terminate it at any time. It is recommended that we discuss termination to ensure a respectful closure. If inpatient care becomes necessary, therapeutic responsibility will transition to the attending staff of the facility.

Fee for Service:

The fee per counseling session at Humantality Counseling is $200. Payment is expected at the time of service, unless alternative arrangements have been made. Payment can be made in cash, venmo, or cashapp. Applicable bank charges will be added for any returned checks. For cases involving insurance, I will provide the necessary information for you to seek reimbursement directly, as Humantality Counseling does not directly bill insurance companies. If assistance with forms or paperwork is required, my services will be billed at $50.00 per hour, with a minimum of one hour.

Appointments/Cancellation Policy:

Each counseling session is scheduled for 50 minutes, and you are financially responsible for all scheduled appointments. A $50 deposit is required when booking an appointment, which will be reimbursed as a deduction from your counseling session fee. The full fee will be charged for canceled or missed sessions without 48-hour notice for a full refund of the deposit. Late arrivals will result in a session ending at the scheduled time, with the full fee applied.

Telephone Accessibility:

Phone calls are primarily for appointment scheduling or short messages. I will make every effort to return calls promptly during business hours. Extended telephone contact beyond basic communication will be billed at the regular session rate of 30 minute increments.

Confidentiality:

  • Therapy sessions at Humantality Counseling are strictly confidential, except in legally defined situations such as threats of harm to self or others or cases of child, elder, or dependent adult abuse. Reporting to the appropriate authorities is legally mandated in such instances.

  • To safeguard confidentiality, I do not accept invitations on social networking websites (e.g., Facebook, Instagram, LinkedIn).

  • Any outstanding debt may be referred to an agency or attorney for collection after a reasonable period, requiring the release of demographic and accounting information. No therapeutic information will be disclosed.

Please acknowledge that you have read and fully understand the terms and responsibilities outlined in this agreement.
